Birth and Death Certificates
If you need to order a birth certificate, you can hit the North Dakota Department of Health web site where you will find links to ordering a birth certificate and a death certificate. As of January 1, 2008, you must provide proof of your identity if you request a birth record. You must be the individual, one of the parents of the individual or you need legal documentation to prove that you are entitled to order the birth record. The form to fill out is located on this website.
